ArboStar provides business management software to tree care companies, and sends SMS text messages on those companies' behalf to their customers, about work those customers have requested.
End users opt in to SMS text messages by submitting a service request or estimate request form on the website of the tree care company they are hiring, and checking a dedicated SMS consent checkbox on that form. Messages are sent by ArboStar on behalf of that company. This page reproduces that consent checkbox and its disclosure language, so that it is publicly verifiable. This is the only way customers are enrolled; there is no keyword, paper, or verbal opt-in path.
Online Consent Form
The form below is a non-functional example, shown for reference only. It reproduces the consent checkbox and disclosure language used on the service request and estimate request forms of tree care companies using the ArboStar platform, where ArboStar is the sender of the resulting messages. The SMS consent checkbox is a separate, dedicated checkbox, unchecked by default, and is never combined with acceptance of terms and conditions.

Note that SMS consent and terms acceptance are two separate checkboxes. The SMS checkbox is optional and unchecked by default. Declining it does not prevent the form from being submitted and does not affect the service the customer receives.
Program Details
Program description. Transactional messages about estimates, appointment confirmations, crew arrival and scheduling updates, and invoices, relating to work the customer has requested. This is not a marketing program.
Message frequency. Message frequency varies, based on the customer's service activity.
Cost. Message and data rates may apply. Mobile carriers are not liable for delayed or undelivered messages.
Opting out. Reply STOP to any message to opt out at any time. A single confirmation message is sent, after which no further messages are delivered unless the customer opts in again.
